→ Notify OCR within 60 days of end of calendar year
→ Maintain log of breaches
>= 500 individuals:
→ Notify OCR within 60 days
→ Notify affected individuals without unreasonable delay
→ Notify media (prominent media in jurisdiction)
Business Associate Agreement
